To find out what your insurance may cover, contact your insurance provider and ask if you have out-of-network mental health coverage. You can provide the following CPT codes for the services you need:

  • Individual Therapy – 90834 & 90837
  • Initial Intake – 90791
  • Couples/Family – 90846 & 90847

You may also want to mention that the therapist is an LCSW to ensure that services with this type of credential are covered. Additionally, you can ask about your deductible, co-insurance payment, number of visits per year, and if authorization is required for visits.

Cancellation Policy

To cancel or reschedule a scheduled therapy session, a minimum of 24 hours in advance of your session is required. Failure to provide timely notification will result in a charge of $75 for the missed session.

Good Faith Estimate of Expected Charges

As a healthcare provider, we will provide patients without insurance or those not using insurance with an estimate of the bill for medical items and services. This estimate, referred to as a “Good Faith Estimate,” explains the expected cost of therapy services. Some important things to know about the Good Faith Estimate include:

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
  • You can ask for a Good Faith Estimate prior to scheduling or at least three business days before your therapy session.
  • If your bill is $400 more than your Good Faith Estimate, you can dispute it.
  • It’s recommended that you save a copy of your Good Faith Estimate for your records.
